Small size is best for medium hands I've used an Evoluent Vertical Mouse (regular size) for years but I still suffered from pain and discomfort in my mousing arm and hand sometimes. This 3M mouse has not completely eliminated the discomfort in my hand but has eliminated the pain I had in my index finger from left clicking.My palm measures 3.25 inches across and I wear medium gloves. I tried both sizes of this mouse and found the small to work best for me so the manufacturer's guideline of suggesting the small mouse for those with hands under 3.5" seems reasonable. I found the the design of the thumb rest on the large mouse to be uncomfortable and couldn't right click without shifting the position of my thumb. I hope this helps others with hands that fall in between sizes. 5Use as a secondary mouse when precision is not required Recently starting getting pain from frequent use of mouse. Most of my work involves casual mouse movements with a little bit of precision work. I typically use all kinds of Logitech mice (M505, M510, M525, M557, T630, V470, and more recently the MX Anywhere 2s and the G502 proteus spectrum), and naturally looked at trackball options like the MX Ergo or the M570. I soon realized that none of them were really going to keep my wrist truly vertical and came across this mouse. I have just stated to use this mouse, but I feel like I can train my hand to use this when precision work is not required. I have both this and my G502 hooked up to the PC and so I can switch to the G502 in a fly for precision work while resting my hands for casual work. This mouse makes things tedius if I attempt precision work, though I will likely get more precise with the positioning and scrolling over time. I have had situations where the right click button has tended to 'stick' as some reviewers have complained, but it hasn't happened very frequently yet.I have fairly large hands (6', 220lbs) and the large wireless fits my wrist well, with a tiny bit of squishing the fingers (and so if it had been a tad taller, it would have been even better). 4Was A 1 Star Until I Adjusted It, Makes It 4 Stars I received this mouse on September 17th. It is now December 19th, so I have given it three months of use before writing this review. Before purchasing this 3M mouse, I was using just your average, typical, mouse with a scrolling wheel in the center. For work I am on the computer around 13 hours a day. Recently my right wrist, forearm, and shoulder started to ache and eventually became very painful because of the unnatural position my mouse held my hand/arm in. Also I use the scrolling wheel very often and my middle finger (which I used to scroll with), started to swell and experience pain in the joints, with a hard, painful bone-like spur on the underside.After using the 3M Ergonomic mouse, the swelling/pain in my finger joint went away immediately, and the bone spur also dissipated.However, while it did alleviate the pain in my wrist, it actually caused more pain in my forearm and shoulder, and now the bonus of the entire thumb area of the hand - from the wrist up to the tip of my thumb. After a week of looking at the design and messing around with folded towels under my arm, it finally dawned on me why this mouse caused so much pain....Though I purchased the 'small', the hand shaft is still too long, and with the toggle button at the top of it, it holds the thumb in an unnatural upward position thereby putting strain on that entire bone. When you lay your arm down on a table, with palm facing inward, in the relaxed position a thumb is NOT sticking upwards, but instead it naturally lays level with the index finger. Hence the hand shaft is too tall.The shaft cannot be cut down obviously, so the 'deck', the part that the hand rests upon, had to be raised. To to do this I took a few sheets of paper towels, folded them to the thickness that would give the best height for my hand, then wrapped sturdy packing tape around it, to keep it folded (and makes it easy to wipe clean). Then I made a 'collar' out of paper towel to go around the base of the hand shaft, to which I taped the front part of the folded paper towels to, it helps keep it from shifting. I then put a large piece of rolled tape on the rest of the 'deck' which holds the rest of the folded towels onto the deck.Did it work?Yes, it did. The paper towels have conformed perfectly to the shape of my hand, and is very comfortable. It stays rather clean because of being wrapped in packing tape, which I periodically wipe clean with rubbing alcohol. More importantly, it has elevated all pain in my arm, shoulder, and thumb area. It holds my hand and fingers in a very natural position.So while it is not perfect, and I shouldn't have to make padding for it, I gave this mouse 4 stars because I do like it, even though I had to make adjustments to make it work. It glides smoothly on a mouse pad. The toggle button is smooth, as is the scroll button. It does take some time getting used to scrolling this way, but once I got the hang of it, it's second nature to me now.Some tips:* I found myself 'gripping' the mouse like a joy stick because I was so not used to a mouse of this shape, which makes the four fingers hurt after a while and would make me hit the scroll button inadvertently. I had to keep reminding myself to relax my hand. No need to 'grip' it, it's not going to take you on a 'white knuckle' ride!* Does not move well on thin or hard surfaces. I had a very, very thin mouse pad and it would not glide well on it. I purchased a thicker pad and now it moves smoothly.* Because the hand shaft stands so far up, I have knocked this mouse right off my desk a number of times, my arm would hit it, or walking past my desk my sweater would catch it. So when not in use keep it away from the edge of the desk. At least I know that this mouse can take a beating! 4Love! I have tried almost every single type of ergonomic mouse device you can think of to find the best fit for my (what I assume is) carpal tunnel. I work endless hours at my desk for work and my wrist becomes extremely fatigued/painful. So far this one has far exceeded all others I've tried. With one small exception.....the scroll feature. For this reason I also ordered the "Penguin" vertical mouse which I had to sell my kidneys to pay for. But am desperate for the perfect mouse. I have been switching back and forth between this 3M mouse and the Penguin trying to decide which I feel has more ease of use and better feel. My dilemma is that I much prefer the 3M in terms of how it feels in my hand and isn't too bulky. But darn that scroll feature! It manageable, but a pain and not precise. I much prefer the scroll wheel on the Penguin. But the Penguin is bulky and not as nice as the 3M. What a dilemma! I guess I'll keep both and just alternate when I get tired of one.All in all...I am very happy with this 3M mouse and it will be my "go to" mouse when I have pain. You will not regret trying it. I also bought mine used as part of their "warehouse deals" and it's in perfect condition. Saved some $ and it was a good buy! If you're on the fence about it from reading other reviews...just give it a try. Worst case you just return it! Amazon is great with their refund policy and it's not much of a hassle. So 2 carpal thumbs up from ME! 4Wireless 3M mouse Great ! I have problems using a conventional mouse. My fingers burn and my wrist hurts. I have tried trackballs and they are better but do not solve my problem.I have had this for two days and the pain is gone and I am very comfortable using this mouse. I was worried at first about the scrolling but after a couple of hours I got used to it and now already it is great ! I would recommend this to anyone that has trouble with pain on a regular mouse. I will be sharing this with all my friends. I bought the large and it is a perfect fit for me. ( I have pretty big hands ). I still use a mousepad. 5Outdated and feature-starved design. Ridiculously priced at nearly $70. Covered in dust. Comfortable, which is the reason I bought this mouse, but they need to get with the times and offer a stay-in dongle that isn't from the 1900s. There's hardly any benefit in making it wireless if you have to find it, plug it in, pull it out, and store it for those of us who move our machines on a twice-daily basis. My friend has used the wired version for graphics work for years, and says the optical accuracy is terrible. Laser has been out a decade and a half.There's also a painfully glaring oversight that compromises its single redeeming feature: no scroll wheel or back/forward buttons. That means extra movement and clicking, which contributes to fatigue and strain. Also means it's poorly suited to most modern programs--it's not good for browsing, it's not good for programs that use shortcuts like Ctrl+scroll to zoom, and it's not even good for writing documents that are over a page long. That leaves Windows Calculator, I guess?I thought I could deal with these downsides before purchase, but I end up only pulling out this beast when I'm IN a lot of pain, not to prevent pain. And even then, it's really too big and odd to use anywhere but a desk. I'm more likely to pull out my Anker ergonomic or my pen-like mouse, but those have some huge compromises as well. Wish Logitech would get in this game.So...if you need it, you need it...just be aware you're buying an ancient $10 mouse inside a $60 hunk of plastic from a company that doesn't care. The nearly identical wired version came out probably 2 decades ago or longer (probably a PS/2 connector originally, for those of us old enough to remember), and they've apparently abandoned development on both models. That wouldn't be so disappointing if there were a lot of great, modern ergonomic alternatives, but I'm still searching.Edit: Good lord, yes, they're still advertising PS/2 compatibility on their wired versions. Amazon was a brand new bookstore on that AOL thingy when their *current* marketing was remotely relevant. 2Took away all my pain but lacks precision I bought this because I have had major wrist pain for 18 years. I've had carpal tunnel surgery in both wrists, with a follow up surgery on my right wrist, and still had pain using a conventional mouse. Let me say that this mouse took the pain away almost immediately! It caused a little bit of pain in other parts of my hand because you are using different muscles to use it, but it wasn't bad and went away in a few days. I can use this mouse every day and have ZERO pain now in my wrist. Based on that alone, I would give it 5 stars. However, it loses points for functionality and precision. I work for a law firm and do a lot of precision work including filling in forms in Word documents that require a lot of selecting, copying, and pasting. I used to be extremely FAST, but I feel like this mouse has slowed me down by about 25% (maybe more). Based on functionality, I would probably give it 2.5 stars. It does not have a scroll wheel, which drives me crazy (although there is a way to "scroll" - just not what I'm used to). But I guess it's a trade off for having absolutely no pain. And based on that, I am giving it 4 stars. If you have had CTS wrist problems like me, where nothing else has worked including surgeries, I would recommend this mouse. If you have no problem using a regular mouse and do a lot of precision work, then by all means go with a regular mouse. I am still on the hunt for an ergonomic mouse that is both precise AND pain-free, and have tried several, but I can't seem to find the perfect one. So far this has been the best pain reliever for me. 4Great for alleviating hand pain, especially at this price. I was having hand pains from using a mouse, and had previously been using a different erg mouse that was shaped like a regular mouse except rotated so that your thumb was facing up. Anyway, my friend was using a joystick mouse and raved about it so I thought I'd try it out.I was shocked to see that a lot of joystick mouses were $100+! But also glad to find this 3M one that was significantly lower priced. I purchased it and was glad I did. It initially takes a little time to get use to using and you may get a few inquiries from your co-workers, but the grip is significantly easier on your hand and since I've been using this mouse I do not get any hand pains.I have had this mouse for about 6 months now and have not had any issues with it. (which I cannot say the same about the prior erg mouse I bought, which I had to replace after a few months).If you're worried that the shape is a little strange, once you get use to it it feels a lot more natural than a standard mouse. Yes, it costs a little more than a standard mouse, but it is totally worth it. The only downside is that the scoll feature is a little finicky. Whereas in a standard mouse you can roll the wheel at the speed you want and stop it at precisely where you want, for this mouse you squeeze the "trigger" button and drag up and down. You can scroll faster or slower this way, but it's not as precise as a wheel. 5Have used these mice for about 10 years I have used these mice for about 10 years. This is the only way I can use a mouse for more than an hour without a lot of pain.The only negative is reliability. I have a very old grey buttoned version that is still going (except right click is flaky). The newer versions I have bought (black button) have not lasted as long. Even considering the reliability issues, these mice are excellent in my opinion.To clarify what some people have asked in the comments - the grey buttoned versions are older and have a lower DPI (but seem really reliable). The newer version has a black button and higher DPI (but may not be as reliable). Amazon's pictures show both the old (grey) and new (black) versions. If you are wondering if Amazon will ship you a old or a new the answer is new (black button). The change came about 5 years ago so the old grey ones would not exist in new form from any supplier at this point. 5There's a reason why both sizes are typically ordered together... Without a definitive guide to selecting the correct size the FIRST time, you're likely going to have to take the Goldilocks approach to discovering which device fits best - unless you know you have small or very large hands.I made the mistake of allowing one reviewer's opinion guide me to the wrong size: "...unless you have gorilla-sized hands, you should choose the small model."Learn from my mistake...The width across my fingers, where I grip the joystick, is around 3.75" - which happens to be 1/4 finger-width too large for the small model, and about 1/4 finger-width to small for the large model. In other words, 3.75 is about a tween size for these two models.At 3.75" - I'm much better off with the large model:I stack my pinkie, ring and index fingers on the base - allow a slight bit of space between my index and fore fingers - and support my thumb on the top of the joystick stem. 5
